Royal Free London NHS Foundation Trust is currently looking for a Band 6 Nurse to join its Adult Hospital @ Home (AH@H) service, with the role mainly based at North Middlesex University Hospital, London N18 1QX. This is a full-time position offering a salary of £45,953 to £54,254 per year. The role would suit an experienced and compassionate nurse who enjoys working closely with patients and wants to make a real difference to their recovery. Rather than providing care only within a hospital ward, the successful candidate will have the opportunity to support patients in the comfort and familiar surroundings of their own homes.
The Adult Hospital @ Home service is designed to provide patients with the right level of clinical care without keeping them in hospital when it may not be necessary. As part of the team, you’ll visit patients at home, assess their condition, provide appropriate nursing care and keep a close eye on their progress. You’ll need to be confident making clinical decisions and responding appropriately if a patient’s condition changes. Good communication will also be important, as you’ll work with patients, relatives and other healthcare professionals to make sure everyone understands the care plan and next steps.
A key part of the role is helping to prevent avoidable hospital admissions while also supporting patients who are ready to leave hospital but still require ongoing clinical care. You’ll work closely with doctors, nurses, therapists and other members of the multidisciplinary team to coordinate treatment and ensure patients receive safe and effective care. This may include monitoring symptoms, administering treatments or medication, updating care plans and escalating any concerns when necessary.
